Documenting Income (be sure to provide for the correct year):
- Provide a signed copy of your Federal Income Tax Return including any of these schedules you filed: Schedules A & C, and 1-3 or
- Provide a Tax Return Transcript (TRT) from the IRS. Choose "Request Online" or "Request by Mail" (allow two weeks). You may also request by phone, at 800-908-9946

FERPA Permission
Enrolled students can give permission to parents, guardians, etc. to access bills, financial aid and/or academic records by completing this online form.
Loan Forms:
- Cancel, decline, decrease or increase an existing loan
- Cost of Attendance / Loan Supplement (when loan is needed for additional costs, 2025-26 academic year)
Satisfactory Academic Progress (“SAP”)

- SAP Appeal Form - for returning students who do not meet the satisfactory academic progress standards and wish to appeal.
