Every student has different needs. We want to take the time to learn all about you and make the best possible accommodation recommendations.
Step 1: Complete Accessibility Services Application Form
- Using your Lake Forest College Email account, login to complete the form on our Accommodate system. Hitting Save, or Submit will submit the form to our office.
- Once your application and documentation have been reviewed, Accessibility Services will notify you to request a Self-Report Meeting.
Step 2: Documentation
- When it is possible, please have your treating provider complete a Reasonable Accommodation Professional Verification Form that may be sent to the medical professional currently treating you for your disability and/or medical condition. This form helps us determine what accommodations are appropriate and reasonable for you.
- You may also submit other third-party documentation (like previous Individualized Education Plans, or previous 504 plans) along with the form to further support your request. For a detailed explanation of what documentation is appropriate, please refer to our documentation guidelines .
Step 3: Self Report Meeting
- After completing the Application Form, students will receive information from an Accessibility Services Staff member to set up a Self-Report Meeting. Self-Report Meetings are approximately 45 minutes, during this meeting your documentation will be reviewed, and you will discuss the barriers you are facing. If it is determined reasonable accommodations need to be put in place, an accommodation plan will be developed.
